Doctor Madness — Psychiatric Text Simulator

Genre & Rating

R-rated dark humor psychiatric text-based simulation game (18+)

Core Concept

You play as Dr. You, M.D., Psy.D., a one-person psychiatric practice managing 12 deeply unstable patients through a secure messaging app. Each patient is a walking DSM-5 entry having the worst week of their life. Your job: diagnose, prescribe medication, and survive the chaos.

Gameplay Loop

Chat with Patients — Select patients from a sidebar and engage in real-time text conversations

Prescribe Medications — Browse 70+ real psychiatric drugs across 10 classes with authentic clinical information

Fast Forward One Week — Advance time to see what your medication actually did (11 possible outcomes ranging from miraculous improvement to overdose)

Manage Consequences — Handle meltdowns, hospitalizations, suicidal crises, rage episodes, and occasionally... improvement

Advanced AI Patient System

Deep Topic Detection Engine:

Patients respond intelligently to 35+ topic categories including medication, side effects, weed, alcohol, hard drugs, cigarettes, sex, music, sports, cars, movies, TV, food, politics, celebrities, animals, planes, gaming, money, relationships, weather, mental health, and more

They understand context-sensitive responses: yes/no answers, questions, compliments, insults, profanity

Each patient has unique opinions, knowledge, triggers, and personality-driven reactions

Dynamic Mood System (0-100 scale):

Kindness, empathy, prescribing → mood improves

Insults, dismissiveness, profanity, challenging their reality → mood crashes

Mood drives behavioral states: Normal, Agitated, Manic, Distressed, Paranoid, MELTDOWN

State changes trigger visual effects (screen shaking, flashing, color shifts)

Personality-Filtered Responses:

Every patient has unique backstory, diagnosis, secondary diagnosis, symptoms, triggers

Aggressive patients explode faster; paranoid patients spiral into conspiracy theories; BPD patients oscillate between worship and rage

Patients remember conversation context and evolve through conversational phases (intro → middle → crisis → resolution)

The 12 Patients

Brenda Smashnuts (BPD + Bipolar I) — Former kindergarten teacher in love with her mailman, oscillates between "you're god" and "I will end you" every 90 seconds

Gary Tinfoilhat (Paranoid Schizophrenia) — Lives in a Faraday cage, tests medications for nanobots, convinced you're a government asset

Deborah Screamsalot (Histrionic PD) — Everything is the most dramatic thing ever, calling Oprah AND Dr. Phil

Kevin Stabsworth (ASPD + IED) — Court-mandated, finds therapy amusing, keeps documentation on everyone

Wanda Mooncrystal (Schizotypal + Erotomania) — Online witch in a relationship with the moon, will hex you

Randy Louderpunk (ADHD Severe) — 11 unfinished businesses, cannot and will not stop talking

Sylvia Cryfountain (MDD + PTSD) — Cried into lattes until fired, watched sad penguin documentary 31 times

Hank Ragequit (IED + NPD) — Banned from six Home Depots, 11 drywall holes he calls "ventilation improvements"

Tiffany Vaporbrain (Substance Use + Dissociative Disorder) — Philosophy student unsure if she's real

Norman Knifecollector (Schizophrenia) — Former accountant mapping "the actuarial apocalypse," 14 cats all named Evidence

Loretta Butterfingers (OCD) — Washed hands 140 times today, types through nitrile gloves

Barry Murmurs (Social Anxiety + MDD) — Rehearsed his first text for 4 days, apologizes for apologizing

Medication System

10 Drug Classes, 70+ Real Medications:

SSRIs, SNRIs, Benzodiazepines, Antipsychotics, Mood Stabilizers, Stimulants, Tricyclic Antidepressants, Atypical Antidepressants, Beta-Blockers, Anticonvulsants

Realistic Clinical Data:

Accurate dose ranges, clinical notes, uses, side effects

Drug-specific patient reactions (e.g., "Lithium? Like the Nirvana song?")

Class-based side effect knowledge (SSRIs → libido loss; Benzos → addiction; Stimulants → crash)

Prescription Writing:

Full prescription pad with dosage, instructions, diagnosis, refills, DEA/NPI numbers

Prescriptions are "mailed" to patients with delivery tracking in Mailbox

Fast Forward Week — Outcome System

11 Weighted Outcomes:

✅ Significant Improvement — It worked. They got better.

🟡 Partial Response — 15% better (tunnel same length, walk faster)

⚠️ Bad Side Effects — Nausea, weight gain, sexual dysfunction, sedation

💢 Rage Episode — Threw a printer, got fired, property damage

🆘 Suicidal Ideation — Black-box warning triggered, crisis line called

⚡ Manic Episode — No sleep, started 6 businesses, spent rent on a boat

☠️ Overdose — Took the whole bottle, currently texting from hospital

🏥 Hospitalized — 72-hour hold, voluntary or otherwise

🚨 Serotonin Syndrome — ER visit, heart rate 160, mixed medications

😰 Withdrawal — Brain zaps, cold sweats, missed doses

😐 No Effect — Pills are decorative

Smart Probability Weighting:

Drug class matters (TCAs/Benzos = high overdose risk; Beta-blockers = safest)

Diagnosis match matters (antidepressants + undiagnosed bipolar = mania)

Dosage matters (high doses = worse outcomes)

Patient personality matters (BPD = elevated suicide risk; ASPD = rage)

Compliance randomization (some take triple dose, some skip weekends)

Additional Features

Auto-Generated Case Notes — Clinical SOAP notes with 400+ real psychiatric terms

Quick Reply Buttons — Therapeutic prompts for faster responses

Mailbox System — Full prescription delivery log

Score Tracking — Consultations, prescriptions written, meltdowns triggered

Crisis Management — Commit button for 72-hour psychiatric holds

Visual Effects — Screen shaking for meltdowns, flashing for mania, particle effects, rain overlay

Sound System — BGM, send/receive tones, notification sounds (mutable)

Technical Details

100% Offline HTML5 Game — No internet required, no data collection

Pure JavaScript — No frameworks, runs in any modern browser

Responsive Phone UI — Authentic messaging app interface

Real Psychiatric Knowledge Base — 100+ diagnoses, 70+ medications, 400+ clinical terms

Sophisticated NPC AI — Context-aware, topic-sensitive, personality-filtered dialogue

Dark Humor & Content Warning

This game uses dark comedy to explore serious mental health topics. Patients use profanity, make death threats, discuss suicide, overdose, hospitalization, substance use, sex, and violence — all portrayed satirically.

NOT MEDICAL ADVICE. The psychiatric information is real; the scenarios are absurd fiction.

Game Philosophy

Doctor Madness is a satirical examination of psychiatric care, medication roulette, patient-doctor dynamics, and the chaos of mental illness — delivered through a lens of irreverent humor, authentic medical knowledge, and surprisingly sophisticated AI patient behavior. It's therapy for people who think therapy is funny. And terrifying. Simultaneously.Game Play
